Welcome to QIIP Toolkits! The following Resource Guides have been designed to help FHTs and CHCs meet their goals in providing high-quality, patient-centered care.

The Needs Assessment Resource Guide is designed to provide an overview, with a focus on needs assessment approaches and processes relevant to the FHT and CHC practice reality.
